CASE MANAGEMENT SERVICES
Case Management Services are designed to help the child and family evaluate different areas of a child’s life to identify not only what treatment needs should be addressed, but also what resources are available within the child, family, and the community to meet those needs.
The case manager is responsible for:
• Maintaining contact with the child and family
• Monitoring the child’s progress
• Evaluating the effectiveness of services being provided
• Determining what additional services would be beneficial
THERAPEUTIC DAY TREATMENT SERVICES
Our school-based counselors provide individual, group, and family treatment. They work in conjunction with teachers and school staff to provide the best integrated care possible to children and their families. The integrated care helps to improve a child's attendance, concentration, behavior, and social skills.
Families work with our therapists to improve:
• Family Communications
• Anger Management
• Stress Management
• Coping Skills
• Parenting Strategies
• Social Skills
INTENSIVE CARE COORDINATION
ICC uses an evidence-informed “High Fidelity Wraparound” model where an individualized team approach moves the child and family to self-efficacy and minimal professional involvement. A key principle of this model is Family Voice and Choice, in which clients are respected as experts on their own families. The family identifies their own needs, selects which professionals and other support people they want on their team and has a leading role in all treatment decisions.
A single, integrated Plan of Care is developed at monthly team meetings held for each family. The ICC facilitation process continues until the team’s goals are accomplished.
