Mission Statement
It is the mission of Case Management to enhance the quality of life for those individuals with developmental disabilities through assessment of needs and preferences, coordination of services and other supports, and empowerment and advocacy.

Description of Program Services
Case Management Services- assists individuals and their families in accessing needed services that are responsive to individual needs. Services include: identifying and reaching out to potential clients; assessing needs and planning services; linking the individual to services and supports; assisting the person directly to locate, develop, or obtain needed services or resources; coordinating services with other providers; enhancing community integration; making collateral contacts; monitoring service delivery; and advocating for people in response to their changing needs.

Hours of Operation
Normal hours of operation are from 8:30 am to 5 pm Mondays through Fridays. |
