What is Case Management?
RACS Mental Health Case Management provides the following to adults with a diagnosis of Serious Mental Illness:
- assessment of needs
- assistance to develop and work on goals
- coordination of services, including referral and linkage to other services and monitoring service delivery and progress
Case Managers often make outreach visits, assist with applying for benefits, and advocate for the client within the agency and the community. |

Who is eligible?
Adults who meet the criteria for having a Serious Mental Illness, including a major mental disorder, two or more functional limitations, and a history of psychiatric hospitalization or expected long-term need, are eligible for RACS Mental Health Case Management services. |

Accessing Services
Individuals interested in this service can call our ACCESS referral service at 463-3141. An ACCESS staff person will take the referral information and assist in scheduling an initial assessment by a qualified mental health professional.
RACS accepts Medicaid and other health insurance plans. Medicaid will pay for most services, including Mental Health Case Management. There may be a co-pay for some services. RACS utilizes a sliding scale fee schedule for individuals with a demonstrated inability to pay our fees. |
