Here are just some of the things that a mental health case worker can assist people with. Case Managers often make outreach visits, assist with applying for benefits, and advocate for the client within the agency and the community. Other services include:
Assesses needs and plan services for adults with a serious mental illness.
Coordinates services with other providers and agencies.
Links individuals directly to services and supports.
Assists the individual directly to develop, obtain and manage needed resources.
Monitors mental status, treatment compliance and service delivery.
Enhances community integration through increased opportunities for community access and involvement.
Makes collateral contacts with significant others.
Provides crisis intervention and stabilization.
Coordinates healthcare follow-up (include psychiatric care).
Offers support for independent living and employment goals.
